A couple of days ago someone posted an email to the Asterisk-Dev list stating that they had been performing load testing of Asterisk and had noticed a problem when more than one voicemail were written for the same person at exactly the same time. This has now been fixed in CVS-HEAD:

It appears that the same INBOX/msg0000 file is created for both recordings. Only one message survives.
I have not tested this in HEAD, but I could not find any previous bugs relating to the problem.
